In this universe Natasha Romanoff had given birth to a daughter before she was sterilized. Her name was Ekatarina. She went through I different version of the red room. She was dusted away with the snap and coming back we follow Yelena who also come back from the snap who is now taking care of 11 year old Ekatarina in which she promised Natasha to take care of if anything happened. For reference with this takes place during the events of Hawkeye.
